Program Statement: Catholic Charities, Migration and Refugee Services (MRS) is seeking a motivated, mission-driven and knowledgeable Social Worker I to provide intensive case management services for refugees, asylees, and survivors. MRS provides holistic, strengths-based, trauma informed treatment that focuses on medical, psychological, social, and legal needs.
You will be a part of a large refugee resettlement office that assists approximately 500 refugees annually from countries including Afghanistan, Bhutan, Burma, Sudan, Somalia, Ethiopia, Democratic Republic of Congo, Burundi, Eritrea, Iraq, Syria, Pakistan, and Ukraine. We respect the dignity of each person and provide a supportive environment where each individual and culture is celebrated. Building upon the courage and strength of these newcomers, we seek to empower them to achieve self-sufficiency and independence.
What you will do:
- Complete biopsychosocial assessments for each client including a comprehensive social history and creating an individual service plan.
- Provide specialized case management including, but not limited to: patient navigation, coordination of specialty appointments, food, clothing, health screening, and public benefits access
- Provide transportation to essential appointments and teach refugees how to travel unaccompanied to future appointments using public transportation
- Complete home visits for isolated, aging, and vulnerable refugees
- Assess clients quarterly and annually using evidence-based measures
- Assist with data collection for quarterly, semi-annual, and annual reports
- Provide clients with evidence-based coping strategies that aid in the reduction of symptoms associated with stress, anxiety, depression, and other mental health issues
- Educate team on social work best practices, stress management techniques, and provide guidance, as needed/requested
- Document all activities with refugees in individual case files; ensure case files are in compliance with agency and federal guidelines
- Prepare reports and maintain confidential records.
